Please note that students are required to bring their own money for lunch & dinner on Saturday. 

Depart: 6:15am from the Peters' house 187 Orchardville Rd, Ayton. Return: approx 9:00pm.

By "submitting" this form, I/We, the guardians of the student listed on this form, authorize or one of the Hope Community Church leaders to sign a consent for medical treatment and to authorize any physician or hospital to provide Rebekah or Paul Peters, a medical assessment, treatment or procedures for the participant named above. I/We undertake and agree to indemnify and hold blameless Hope Community Church, it's Pastors, Representatives and Board Members from and against any loss, damage or injury suffered by the participant as a result of being apart of the activities of Hope Community Church as well as any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in or travelling to "Today's Teens Conference, Brampton, 2026".

I/We also agree to grant permission for the reasonable use of pictures containing our student in any or all of the following ways: Website, Social Media, Promotional material.
