By submitting this registration, I hereby grant permission to Hope Community Church to take photographs and/or video footage of my child while attending the Momentum Conference. I also hereby grant permission for the reasonable use of photos/videos for website, social media, and promotional posts.

By submitting this registration I, the parent/guardian, give permission for my child to participate in the Momentum Conference 2026 with Hope Community Church and I assume the risks that may accompany such participation.

Precautions are taken for the safety and health of your child, but in the event of accident or sickness, Hope Community Church, its staff, and its volunteers are hereby released from any liability. In the event that your child requires special medication, x-rays, or treatment, the parents/guardians will be notified immediately.

By clicking "submit" I agree that Hope Community Church, it's directors, employees and volunteers should not be held liable for any loss, damage or injury to my child or my child's property arising or resulting from participation in the Momentum Conference 2026 with Hope Community Church. I give Hope Community Church officials the authorization to act on my behalf in the even of an emergency.

This consent and authorization is effective only when participating in or travelling to and from the Momentum Conference 2026.

By submitting this form, you are agreeing to have a ticket purchased for your child. If your child is unable to attend the event, $80 will be non-refundable (unless we can re-sell the ticket).

Please fill out this form and click submit for your child to attend the Momentum Conference on Nov. 6-7, 2026. If you have multiple students, please register each with a separate form. And complete all form questions for each student.

The cost will include the conference ticket, transportation, overnight stay, and food (except Saturday dinner). Please send money for dinner on Saturday and conference merch (if wanted).

If you are able to cover the full amount, please send in payment for $125. You may pay by cheque (memo Momentum Conference & Student's Name), e-transfer to giving@hopecc.ca (memo Momentum Conference & Student's Name), or by cash to Pastor Adam White.
