By submitting this registration I, the parent/guardian, give permission for my child to participate in the Hope Kids Wednesday Midweek Program at Hope Community Church for the 2026-2027 year and I assume the risks that may accompany such participation.

Precautions are taken for the safety and health of your child, but in the event of accident or sickness, Hope Community Church, its staff, and its volunteers are hereby released from any liability. In the event that your child requires special medication, x-rays, or treatment, the parents/guardians will be notified immediately.

By clicking "submit"  I agree that Hope Community Church, it's directors, employees and volunteers should not be held liable for any loss, damage or injury to my child or my child's property arising or resulting from participation in the Hope Kids Wednesday Midweek Program at Hope Community Church. I give Hope Community Church officials the authorization to act on my behalf in the even of an emergency. 

This consent and authorization is effective only when participating in or travelling to and from events of Hope Community Church.

The following form must be filled out by a Parent/Guardian (18+) of the child participating.

I have read, understood and agree with the above form for the Hope Kids Midweek Program at Hope Community Church. A seperate informed letter of consent will be sent home for off-site activities and activities of elevated risk.
